Codex keeps failing with “stream disconnected before completion”

Hey everyone,

I'm having a persistent issue with Codex where requests fail with this error:

stream disconnected before completion: An error occurred while processing your request. You can retry your request, or contact us through our help center at help.openai.com if the error persists.

This has been happening since September 16, 2026, and I haven't been able to get Codex working again.

I've already tried pretty much every troubleshooting step I could think of, including:

• Restarting VS Code
• Restarting Windows
• Signing out and back into OpenAI
• Checking my Internet connection
• Checking DNS resolution
• Reinstalling/resetting the Codex extension
• Clearing Codex/VS Code-related caches
• Trying different conversations/requests

Nothing has fixed it.

I'm using Codex in VS Code on Windows. At this point I'm wondering whether this is a server-side issue rather than something on my machine.

Has anyone else been experiencing the same error with Codex recently, especially since September 16?
