Cook County 1930 marriage license, what’s actually on it?

I have an ancestry.com index entry from the Cook County Marriage Index 1930-1960. License no. 1293717, married 10 Oct 1930, groom Boyd Weil Smith, bride Etta Mae Marshall. That's all the index gives me. No ages, no addresses, no parents.
I'm trying to work out whether this is the right man, so what I need from the actual record is his age or date of birth, birthplace, home address, occupation, parents' names, and whether it was down as a first marriage.

So my question is whether the 1930 Cook County license application actually has all that on it, or whether the surviving record is just a line in a register. There's a $15 search fee and I'd rather not pay it to get back a document with four fields on it. If anyone has ordered one from around this period I'd appreciate knowing what you got.

Second thing. genealogy.cookcountyclerkil.gov is throwing a Cloudflare 1016 DNS error. A couple of research guides list the Clerk's historical vital records site as not currently available, so I'm guessing this has been broken a while. Is the mail-in Genealogy Record Request Form the only way to do this right now, or is there a working
URL I haven'ts found?

Last one. The name search on FamilySearch turns up nothing for this marriage. Has anyone had any luck browsing the Cook County marriage films directly by license number range when the index comes up empty?
